How you collect phone numbers is scrutinized heavily during the A2P Campaign review process. If your GoHighLevel forms aren't compliant, you will not get approved.
1. The Consent Checkbox
You must have a checkbox on your form for SMS consent. CRITICAL: This box must be unchecked by default. Pre-selected checkboxes are an automatic rejection. The user must take affirmative action to opt-in.
2. Clear Disclosure Language
Next to or below the phone number field, you need clear disclosure language. For example:
"By providing your phone number, you agree to receive text messages from [Your Business Name] regarding your inquiry. Message and data rates may apply. Reply STOP to opt-out."

Your form area should include hyperlinks to your Privacy Policy and Terms of Service. Reviewers will click these links to ensure they work and contain the required language.
4. Optional vs. Required
While you can require a phone number to submit a form, you cannot force SMS consent as a condition of service. The SMS consent checkbox itself should technically be optional, even if the phone number field is required for a phone call.
5. Screenshot Proof
When submitting your campaign, providing a direct link to your opt-in form is best. If the form is behind a login or part of a complex funnel, you may need to provide a hosted screenshot URL showing exactly where and how consent is collected.
